Illinois Primary Health Care Association

Established in 1982, IPHCA is a nonprofit trade association of community health centers (CHCs) that operate more than 350 primary care sites statewide and in neighboring Iowa. These CHCs serve in excess of 1.1 million patients annually. IPHCA strives to improve the health status of medically underserved populations by fostering the provision of high-quality, comprehensive health care that is accessible, coordinate, community-directed, culturally-sensitive, and linguistically-competent. Ultimately, IPHCA works to increase access to high-quality, cost-effective primary health care services in urban and rural populations throughout the state, regardless of an individual’s ability to pay.

 

  

 


 

The Illinois Health Connect program is a primary care case management (PCCM) model where each eligible person has medical home through a primary care provider (PCP).
